We are excited to announce the launch of our Hospice at Home pilot, a program designed to bring compassionate, quality end-of-life care directly to individuals in the comfort of their own homes. During this pilot phase, our dedicated hospice team provides in-home services and care to meet each patient’s needs, focusing on comfort, dignity, and quality of life. While on the program, you can expect our compassionate, expert healthcare team to arrange regular home visits and calls that work with your schedule, while also being able to have responsive support when you need it.
Nursing services are available with Registered Nurse in home visits between 7am and 11pm, as well by phone or the virtual call-bell 24/7 and are able to visit in person as you need them.
Expert care and support is provided by our dedicated Palliative Physician Team, who is available and on-call 24/7.
Care aides are able to support both on a scheduled basis, as well as on an as needed basis during the day and evening hours.
Additional supports are available to the client and their loved ones as needed, such as Social Workers, Counsellors, and Volunteers.
