If you, or a loved one, has received health care news of a life limiting illness, or you are proactively preparing for the future, NOHS’ Resource Navigator can help you access the local services, information and supports you need so that you can be in charge of your journey. The Navigator will meet with you individually, or with your family, by phone or virtually to help you gain the resources you need to make informed decisions.
